Insurance Status Update

 

Evolve Counseling & Consulting, LLC does not participate as an in‑network provider with any insurance plans. All services are rendered on a self‑pay basis, and payment is due at the time of service.

Out‑of‑Network Reimbursement Clients who wish to seek reimbursement from their insurance carrier may request a superbill, which includes the required service codes, dates of service, and provider information. Submission of a superbill does not guarantee reimbursement. All determinations regarding coverage, out‑of‑network benefits, and payment are made solely by the client’s insurance carrier.

Client Responsibility Clients are responsible for verifying their own out‑of‑network benefits, understanding their plan requirements, and submitting any documentation directly to their insurance carrier. Evolve Counseling & Consulting, LLC does not communicate with insurance companies, complete authorization requests, or provide additional documentation beyond the superbill.

Financial Policies By engaging in services, clients acknowledge and agree to the self‑pay structure and understand that insurance reimbursement, if any, is between the client and their insurance carrier.
